When scheduling a new patient appointment the same day, please allow enough time to fill out intake forms online. **Intake forms must be completed prior to your appointment.***Intake forms will be sent via link in email and depending on your health history can take up to 20 minutes to complete. After scheduling, check your email. You will receive 2 emails. The first email will contain your appointment confirmation and the second email will contain a link to fill out intake forms prior to your appointment.
